Casey from SWISCO responded:
You may need the 1530 stamp option, to check remove the window sash and place it on a scale. The weight of the window sash is needed to identify the proper stamp option.

1510 will carry a window sash weight range of 4 through 6 pounds.
1520, 6 through 10 pounds.
1530, 10 through 12 pounds.
1540, 12 through 13 pounds.

This information is based on 2 balances per sash. Balances are designed assuming friction in the jamb weather-strip will carry a portion of the sash weight. Suggested sash weights are approximate.
A contractor from Parker, Colorado says:
The S370-16 product pictures look exactly like what I need to replace, but your options don't match what is stamped on these balances.

Some are marked: 15 3 CALDWELL AN
Some are marked: 15 3 CALDWELL JM

Are these special orders? What p/n should I order and what would the price be?

Thanks! Jim
