5/8" Top Sash Guide

Need help identifying this 5/8" x 1-3/4" top sash guide. We have identified the channel balance replacement as the 385 series but I'm having difficulty identifying this part as it appears that part of it is broken off top face (small square).

Hello! I recognize this top sash guide. I'm afraid this exact model is no longer made, but we've found that our 18-171 is a possible substitute. The only issue is that the top is tall, so you would need to cut it down to fit under the sash rail.
