Channel Balancer Identification

A home owner from Ashland Oregon says:
My main question is that the length of the channel is 25 5/8 inches so I am not sure what size to use.
Secondly it appears that the hook is lower on the change then those shown.
From the pictures it appears to be a 19-005 top and a 15-005 shoe.
These are Milgard non-tilt. The window weighs 19lbs
The channel is 1/2 inch wide
The shoe length is 1 inch

Thanks again for your patience. We can replace this balance, but it will need to be custom made. That shoe is not the 15-005, but a unique type that has the pulley wheel built inside of it. Before we begin, I will need to know how m any you need.
