Window shoe from Comfort World Brand double hung window. Likely comfort world 4000.

Pivot cam is the only piece that has failed. Pivot cam is 5/8" diameter. Pivot cam depth is 1/2" Top to bottom length of shoe is 1 7/16". Width is 1 1/4". Depth is 1/2".
