Help identifying parts for Caldwell 28 4 CO

A quick learner from Lexington, KY says:
I'm not sure which balance I need for a Caldwell 28 4 CO. I think it would be 19-005 top, but not sure on the shoe. Maybe 15-005? so Series 380? Please see included photos.

It is for 29" length and 1/2" width.

Thanks for posting. Based on these photos, I think you should consider our S380-29 channel balance. I would also recommend the 2840 stamp option, for sashes weighing 23 through 28 pounds.
