Malta pivot shoe identification request

A home owner from Leominster, MA says:
Attached photos are from a Malta, double hung, vinyl, tilt window. In four of my windows, the pivot shoes have broken/fallen apart. Please help identify the part and suggest a replacement. My Malta windows were produced in Feb 1999. Jamb is plastic and channel balance dimensions are 1/2" x 1/2" The 15-159 Malta pivot shoe you offer is the right depth, but too narrow for the channel - it "wiggle" in the channel, or at least does not appear it will properly seat in the channel. In the channel, an approx 25" spring hooks over the top of the channel, and on the opposite end, the pivot shoe hooks onto the spring. The channel is ribbed so that it embeds into the jamb, and only 5/16" is revealed. The pivot shoe hole is 3/16". I looked at replacing the balances with one of your 570 series but couldn't find a pivot shoe to fit the channel. Thanks for your help.
