Need to replace Channel Balance & Top Sash Guide

A quick learner from Fort Myers, FL says:
It is marked "25 3 BT Caldwell". The metal length is 26" and it is 1/2" wide. The window is a "PGT model SH 4000" and is about 15 yrs old. Window opening size is 52" wide x 55" tall. I think I need a S395-26 and a 18-009 guide? I've tried to send with pictures, but I get error message.

Tom from SWISCO responded:
Is the top sash guide in your second picture the only thing that's broken? If so, you may not need to replace your channel balance. The only way to know what top sash guide will work is to carefully compare your measurements to ours. If the 18-009 dimensions differ in any way, let me know exactly how. You could also consider the 18-015.

The same goes for the balance. The S395-26 is a possibility, but your picture only shoes us the top attachment. I cannot confirm if the S395-26 is a match based on that alone. Look over our pictures and measurements and tell me if they're not the same.
