Operator Needed

A handy person from Jupiter, FL says:
Please help me identify the replacement part needed for this operator. Came off a Pan-Am window. Top screw holes to bottom screw holes ~ 5 1/8" center-to-center. Also, need to identify and order screw to atrach arm to window since threads stripped while removing. Thank you.

Dave Sr. from SWISCO responded:
Take a look at our 39-017 operator and compare the hole placement with your operator. Also look at the 33-067 screw and 33-207 washer.
