Operator w/hardware

A contractor from Silver Spring says:
We are looking for the Operator, cover and 3/8" folding crank handle.
the numbers on the back of the operator are c-1081-201. the longest arm is s-1081-202. on the smaller arms are s-1081-03 and s-1081-66.

Hmm... this is a strange operator. We don't have an exact match, but I'm wondering if our 39-032 could work as a replacement. You would also need new hardware to go with it, like our 39-107FK and maybe even a new stud bracket like our 39-509. What do you think?
