Great. That does look like the Series 377 balance, yes. Check out this request form for the 11-1 Channel Balance and this other for the 11-2 Channel Balance and fill out the rest of the information for both. I'm not as sure about the glazing. It's hard to say. You could consider our 55-006 as a possible replacement, though I cannot guarantee it will work.
