Replace casement operator

A handy person from Long Island, NY says:
I have been trying to find a replacement for this operator for my kitchen casement window (on the right) with same one (or a universal that will fit).
The only marking in underneath the screwplate it reads "ACTIVE CO.N.Y." Otherwise the closest one I can find is the "Truth Dyad Operator" 39-315 on your site. Will it fit? Another concern I have is that the part of the arm that connects to the window has an upward bend in it. Yours looks straight.

Cover: 3 3/4"W x 1 3/4"L x 1/2"H
Bottom Screwplate: 3 3/4"W x 2 5/8"L (3" spread screwholes)
Gear Arm: 3 1/8" at bend
Window Arm: 5" (1/4" screw hole at the end)
