Replacement anwing window opening/closing mechanism

A quick learner from Middleton WI 53562 says:
The hardware indicates TRUTH 45002 and the arm indicate Truth patent # 3,457,675. The arm length measures 11-1/8" from center of pivot hole to center of mounting hole-widow. Arm length from gear end to arm end is 12".
The base is 5" wide x 2-5/8" deep. The base has 4 mounting holes, the exterior holes are 4-1/8 apart center to center and the interior holes are 2" apart center to center.
