A handy person from Waldo OHIO says:
I have several 1959 windows ,top hung with cranks that have slide bars attached to the bottom of the window. the opening in the frame is 4" wide and they attach with three screws through the mounting plate into the bottom of the frame. do you have any idea if you have a crank that matches this

Your operator is quite old. You likely won't be able to find an exact match. That said, take a look at our 39-408. This has a sliding shoe at the end that appears similar to what you have at the end of your operator's arm. Could this work the same way? Could you adapt it in some way?
