Sash Guide Help

A quick learner says:
I have a broken channel balance but was able to find that part (S351-31), but I'm having trouble finding the correct sash guide. I think it may be 18-018, but not certain. Can you take a quick look at the attached and let me know? I'm not sure on the make/model of the window - it's an older home built in the 80's I believe.

Also - there was only 1 channel balance in the window when we bought the place. I'm assuming there should be 2 - one on each side. Is that assumption correct?

There should be two balances on each side of the sash, yes.
