Snap In Plastic Glazing

Hi,
We are trying to repair the plastic snap in glazing on our windows. I don't know the make or model of the window, but I've tried to include measurements and pictures to show what it looks like. If you could point me to the parts, that would be great! It's a single-hung window, double paned glass I think.
