Unique balance with heavy duty hook

I'm am looking for two window balances and a pivot shoe for a Bonneville window. The balance is a Unique 29-D-40, it is 30" long 19/16 wide and 17/32 deep. The sash weighs about 23lbs. I believe I need a S770-30 with a 30-D option, however my balance has the heavy duty pointed hook 62-105, so I would need two of those as well. It's my understanding that would be a special order, so I want to find out how much would it cost to get those parts as well as 1 Pivot Shoe 15-208?

Thanks for posting. It looks like the pivot shoe is 15-208 for the Bonneville window would work. The balance will be a custom S770-30 with 62-105 hook attached and weight option 30D.
