Your pivot shoe resembles the 15-210 or 15-231 shown below. Both shoes are used with the 15-225 Tamp-R-Lok.

As for the pivot bar, we do not carry an exact match but we have recommended the 26-241 as a possible replacement in the past depending on how the dimensions below compare to an undamaged version.

I can't be sure of the channel balances that you need but the S770 would be a good place to start. You would need to confirm if yours installs with a screw at the top and uses the same terminal clip at the end of the pulley clip that attaches to the pivot shoe. If so, I would recommend weighing your window sash and following the chart under the balance product description to confirm the corresponding strength option. Thank you!
