Block and tackle balance help

A home owner from Los angeles says:
I have a BSI 3130 block and tackle balance, 5/8" deep, 5/8" wide, 32" long. I'm not sure what to replace it with.

Thanks for your photos. Based on what I see here, you have either our S380-32 or S385-32, depending on the depth of the bottom shoe. That's the only difference between them. I recommend measuring your shoe and referring to our dimensions to confirm the best match.

The top piece on my existing part is not tapered like photos show for the S380-32 and S385-32. Will that make a difference? Will I need to also order a matching sash guide?

No, that's never been an issue in my experience. It's just a matter of your balance having been made by a different manufacturer. Ours still works the same way and will be compatible with all the same exiting hardware. Once installed, you won't notice a difference.
