Casement operator

Hello! Looking to find a replacement for this casement operator along with the arms. It has no number markings on it, but does say "truth" on it. Measurements are : left arm (12 inches), right arm (5.5 inches), operator (4 inches wide).

Please start by reviewing the style and dimensions of the 39-013 operator. We do not carry a 12" arm, but please refer to the diagram below to re-measure from the middle of the gear to confirm a good fit! 

The operator only has one arm. I believe 5.5 arm being referred to in the picture is the hinge assembly. Do you need new hinges as well? Please let me know!
