We should be able to assist with a replacement option, but we will need to see pictures of the bottom of the balance where we can see any attachments. Please also confirm the length of the metal channel. From there we can determine the series your window uses. Thank you!

Please review the Series 795 channel balance as a replacement option. The 26-5 stamp should translate to the 26E stamp that can support a 26 to 30 lb window sash, but we recommend weighing the sash to confirm this before purchasing. 

As for the T-lock shoe, the S795-26 comes with the 15-195 closed cam shoe, but I cannot tell from the picture if yours has a closed or open (see through) metal cam style. If yours does have the open style, you will need to purchase the 15-195B separately and switch it out before installation.
