Our 18-202 resembles your old top sash guide. Have a look at the dimensions and see if they match up.

We're happy to help with the balance but we will need to see photos of it removed from the window. We must see the top and bottom fittings as well as the inside of the metal channel where the innerspring connects to the pulley mechanism.

It appears you have either our Series 351, with a 15-002 bottom attachment, or Series 390, with a 15-001 bottom attachment. Take a look at the image below to determine which balance you'll need. 

Are these balances from the same window? It appears the one removed has a 33 2 stamp while the original pictures shown had a 33 3 stamp. The stamp should be the same on both sides. If these are from the same window, can you provide the weight of the window sash?

Are both balances shooting up past the sash? The original pictures does show a damaged guide, but the newer pictures it does not look as damaged. It could be worn down. Let me know!

33 3 34" long bottom plastic piece 11/8 wide

Window guide 18-202 Jordan

Channel balance appears fine. One channel rod always shots up. Appears that the frame maybe 1-2 mm wider than it should be. So the window doesn't catch on both balances.On some I'm able to make it catch by pushing the sash to the side that isn't catching with a stew driver

Advice? What needs to be replaced

Looking at your photos of your window frame, it doesn't look like you can shim the master frame to meet the sashes. I suggest adding a washer at the top and a bottom screw behind the top sash guide, the 18-202. This should prevent the balances from shooting past the top sash guide.
