Thanks for writing in! Your handle set reminds me of out 82-086 Legacy Keyed Handle Set. This handle uses the 82-107 Sliding Door Latch Lever Kit. Take a look over it to confirm that it would be compatible with your handle. I am confident this is the replacement you'll need.
