Hello there! Our S795-34 with the 34C option appears to match your balance. This comes attached with the 15-195 closed cam shoe by default but if yours has an open cam, see the 15-195B instead which is sold separately. I also want to mention that the Series 795 installs at the top of the window track with a screw. If yours uses a hook, please let me know!
