Hi there! I recognize this. It's an old and discontinued balance but we can still help via custom order, though our replacement won't be the same as what you show here. To outline it for you, we'll be using a "skinny" ribbed version of our Series 795. This balance comes with a T-lock shoe by default but you won't be able to use it. You'll need to pop it off and find a way to re-install the metal clip that anchors to your pivot shoe for our replacement to function. Your old metal clip is obsolete so it's important that you hold on to it. For us to get this moving for you, can you confirm the number of balances you'd like to order? Can you also tell us or show us a photo of how the balance installs at the top of the window track? As for the pivot shoe, see the 15-134. This looks like it could be a good match but I recommend you compare the specifications below to confirm.
