The stamp printed on the balance doesn't help us identify it, unfortunately. Can you remove an undamaged balance assembly and show us some photos? The related video on this page will go over the details we're looking for.

Thanks so much for the photos! They're just what we needed. My recommendation is the S395-23 with the 2230 option. Take a look at the specifications and let me know if you have any questions. 

It sounds like you're referring to the top sash guides. We'd be happy to help with that but the balance's identity doesn't tell us which one you may have. For us to help, we will need to see an undamaged top sash guide removed from the window. Seeing the side profile and a shot of the front of the guide would be a great help as well.
