A handy person from California says:
16 inch tube with spiral mechanism and a plastic shoe. Tube has one top screw that connects to window channel frame. Plastic boot is partially broken in picture due to the screw rusting. The plastic boot screws into bottoms of the window frame. Window is removed by pushing to one side and lifting other side out.

Your spiral balance looks like a version of our 5/8" diameter S500-16, with a red tip. However we don't have a version of this with that same nylon boot. It looks like the window manufacturer might have custom made it; if so, this isn't a project that we can help with. 

We do sell the 15-040N nylon shoe that might work in the same capacity, but it's still so different that I can't really say for sure. I'm not totally sure. Besides the back being 5/8" wide, the height 1-3/16"and 3/8" thick, the wedged comes out 1" from the back to the lip. Being nylon, you can drill a hole in the lip to attach the shoe to sash. Look it over and see what you think.
