Hello, thank you for posting,

Your balance resembles the Series 795 channel balance, but with the 15-194 T-lock shoe installed and the 15-198 hook at the top for installation. This configuration will require a custom order to assemble for you as the stock Series 795 uses a screw tab at the top instead.
