Thanks for writing in! I believe our 18-009 5/8" Top Sash Guide may work as a suitable replacement for your existing top sash guide. Unfortunately, we do not carry an exact replacement match for the guide you have. As you'll notice, this replacement guide does not have the tabs on the side like yours does but it should still work well as a suitable alternative.

Take a look over the dimensions and see what you think.
