Active door is a swing bolt that retracts the concealed flush bolt from the threshold. At the same time it retracts the concealed flush bolt from the header on the inactive door. This flush bolt is somewhat bent whereas the swing bolt jams against it rather than underneath to throw the bolt. These are entrance doors (Kawneer)

The flush bolt is contained within the frame of the door. Cannot get a photo. When locking door the swing bolt engaged this part underneath and makes it travel into the header. It was misaligned so when the customer locked the door the swing bolt got jammed behind the flush bolt to the point the door could not be unlocked with the key. There are two screws that appear to secure a guide so the flush bolt down not randomly move. The bolt is about 30 inches long.
