We'd be happy to help but, unfortunately, the numbers stamped on your balance do not indicate the correct replacement. We should be able to help but we would need to start by seeing detailed photos of an intact balance removed from the window, including both ends and the clip at the end of the pulley cord. Thanks for getting back to us. With this combination of attachments, you could have either the S380-20 or S385-20 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

Unfortunately, your stamp code is from an outdated weight system for these balances, so you'll need to weigh your sash to determine which option you'll need.
