hopper style window hinge

A contractor from Huntington Beach says:
I've attached some picture of what the existing hinge looks like. The building was probably built in the '60s. There's a plastic guide that is broken on a be side but I would assume the whole hinge would need to be replaced. On said guide there are some numbers that I'm just now seeing after I zoomed in on the picture, I'm not sure if it's a partial number or what's needed at all. That was the only numbers that I could find on the hinge. The track the bar attaches to at the top and that the guide slides in is 40 1/2" in length(height) and has a width of about 1 1/8". I appreciate your time and any help in identifying these parts/pcs.
