Hello there! Thanks for reaching out to us. It's hard to say for sure since your top sash guide is damaged but the 18-009 comes to mind. I recommend you review the specifications and compare them with an undamaged version to confirm.

Before I make my recommendation for the balance, can you show us a photo of the inside of the metal channel where the innerspring connects to the pulley mechanism?

Thank you for the additional pictures,

With this combination of attachments, you could have either an S380 or S385 channel balance. You’ll need to measure as shown below. The Series 380 has a 1" bottom attachment, and the Series 385 has a 1-3/32" attachment. 

The length of the metal channel excluding the nylon attachments should measure 27" long and you will need a 2630 stamp option. Please review and let us know if you have any additional questions.
