Thanks for reaching out! Your spiral balance looks like it could be our S500-24. Our tip colors are red, blue, and black so I am unable to determine which option you'll need for your sash. With that said, to determine your tip color, you will need to obtain the exact weight of your sash. There is a chart below the product description of the balance that list the weight ranges each tip color can hold.

Your bracket appears to be the 17-012. Take a look over the dimensions to confirm that this is the right replacement. I am confident this is the bracket you'll need.
