series 765 replacement block and tackle channel balance

I believe these were part of several thousand windows installed on Hovnanian homes around 1999-2000. I need a replacement as one has broken on my window. I want to order the part but don't know if this part is correct and what the Stamp Option is. Please help.

I would be happy to help. To confirm the proper balance, please remove it from the window and take a couple pictures of it for us to see. Please also show me the stamp printed on your current balance's metal channel. If you have no stamp, we will need you to weigh your sash on a scale and give us the result.
