I don't believe you will be able to locate the exact commercial door hinges you need for your door, unfortunately. I have two ideas though. One would be to try to install the 79-121 pivot. It won't be easy and you will need to drill new holes to match up to the new hole locations. You will also need to cut the threshold at the bottom to accept the new lower pivot. Another idea would be to install a "continuous type hinge" like a "Roton hinge". I would suggest contacting a locksmith or a glass shop in your area to do the installation.
