This looks like an old Miami awning operator. These aren't made anymore, but look at our  39-054 operator to see if this can work on your window. They aren't quite as heavy duty as your own, and the installation holes might be off a little, but I believe that they are the closest that we have. Please compare to your own hardware and let me know what you think.
