Broken bifold cabinet door hinge

I may have already sent this, but the note I was writing suddenly disappeared. Forgive me if this is a duplicate.

This hinge (#1) broke recently and I am looking for a replacement. The measurements are 1 3/8 across each piece and 2 1/2 (or 2 5/8) long. Picture #3 is the only identifying markings I can see on the piece.
