Door is scraping bottom, occasionally

Looks like you need a new commercial door pivot, though I don't know exactly what this is based on your photos. I recommend looking through our complete selection here to determine if any of our parts match up.

Going to try again. This is inside looking up at top hinge. Screws are 5/8" apart...center to center. When I'm inside, the hinges are on the left and the door swings to the outside. I think it's a flush mount? What would you recommend?I need top and bottom.

I believe the 79-121 pivot set is the one that best matches the one you need to replace. Take a look at the information we show on its store page and see what you think. 

Take note on installing the lower threshold pivot: I would leave the angle portion of the pivot in the door frame. Best to drill out the screw heads if you cannot unscrew the bottom threshold part.
