A quick learner says:
Stamp on the short arm reads: S-1081-66. Base plate is 3 1/2 " wide. Maybe Andersen windows?

This looks like an older style Truth brand operator. I'm afraid we cannot offer this exact part, since I'm not sure it's made anymore. You may be able to get our 39-031 and 39-032 to work, along with a new 39-107 cover, but these both have a much wider base. If they can work, and I'm not sure they will, then you'd almost certainly have to alter the sill trim to accept the larger size. 

I'm sorry that we can't offer a closer replacement option.
