Need BSI Window Balance help

Hi!

My parents need a couple of replacemet slides for an 18-20 year-old single hung window. The one in the attached photos doesn't properly retract, and simply hangs.

Based on my research, it appears to be a 29 inch BSI 60G Extended Balance, but the top end has two "shelves" and doesn't seem to match any I've been able to find online. Also, the hook may be discontinued.

Could you please tell me if these are still available, and if not, the proper substitute?

My parents have three windows that use this balance, but only one is regularly used.

They're going to need at least two balances, and depending on the price, we may order a couple of spares.

Could you please give me pricing for 2 and 4 units?
