Judging from what I can see of the arm, I believe you have an Andersen operator. See either the 39-094 and 39-095, or the 39-150 and 39-151. The main difference between these two sets of operators is the length of the arm, so compare both with your old hardware to see what matches best. For a new handle see the 39-286
