The balance needs to be replaced so i am looking to buy a new one. i have attached the upc off of the window. The slop is measure at 8 degrees( if needed) the inner track is 35 and 3/4" and the outer track is 36".

Your balance resembles our Series 751 channel balance, but you will need to remove the balance to compare the overall dimensions of the balance and shoe attachment. Please view the below video for more information and let me know if you have any questions!
