Replacement Channel Balance (custom)

I need a channel balance for our window and can't seem to find one that meets the criteria of the window we have (see photos of channel balance):

Channel length: 12 and 3/8
Shoe PAT: 6598264
Height of window with sash: 15 and 5/8
Height of visible glass: 12 and 1/8
Sash Weight: 10.5lbs
Number of channel balances: 2

We can replace this balance, but it has to be custom made due to its unique length and design. Can you show me the stamp code that should be on the metal channel? Can you also show me the bottom nylon fitting or shoe from a side-profile angle? I want to see how it attaches to the metal channel. From there I should be able to prepare a quote for you.
