See attached images. My main issue is failure of the internal spring that returns the handles to the latched position.
SWISCO 40-194 is very close, but holes to mount unit to edge of door are of different spacing (6 7/8" rather than 7"). Guess I could just use old mounting plate if you can sell the mechanism without the plate. What do you think?
FYI the backside of the mechanism I have is very close/identical to the image submitted by another on November 16, 2021.
Also could use clip that retains outside handle and the little nylon "washer" that sits between handle flange and cover, if available.

Hi, Jeff.
I'm afraid your pictures didn't upload. I'm sorry about that! Could you try re-uploading them?
Thank you!

Thanks for the re-upload, Jeff. I do agree that our 40-194 looks like a great match, but we unfortunately don't sell this with a different type of mounting plate (or even without a mounting plate). That said if you can reuse your original you could always just discard the plate the 40-194 comes with. It's not permanently affixed to the mortise or anything.
