I think I might know what your balance is, but based on this photo it's a special configuration that we have to custom make. Now, custom balances can't be returned for a refund, so we better be thorough just to be safe.

To start, please remove an undamaged version of this balance from the window and show me a clear picture of the top end, bottom end, and stamp. I especially want to see how it attaches to the window track at the top, be it a screw or a hook. I will also need you to tell me how many you were looking to purchase. Finally, can you tell me if you also needed the pivot shoe? If yours isn't damaged then you can keep reusing it, but we do sell it separately.
