How do I know what part to order I have no clue of a part number.

The only way to determine the proper replacement is to go by the physical design of the original part. In your case, this balance strongly resembles our Series 751. However your bottom shoe is a little different. I think it might be damaged.
Is that accurate? If so, do you have an undamaged version we can see, just to be sure that my pick is right?
