Not sure what this part is called. I need a left and right side. It's like a dust cap that also holds tip of balance in place.

Thanks for posting. That part is called a top sash guide. As you guessed, it keeps the channel balances aligned with the window sash. Without seeing the whole guide I can't be sure, but based on the shape of the cap this might be our 18-124P. See the diagrams below, compare with your old guide and see if this matches up.
