Balance and Shoe for Unknown Builder Grade Tilt Sash

A handy person from Marlton, New Jersey says:
Hello!
I bought a house built in 2002 that has all original windows. I'm not sure of the manufacturer or anything, but I have attached everything I have found since the problem occurred. I closed the window and heard a loud crack and pop noise. The balance was at the top of the tracking the plastic pivot shoe had broken into pieces. I'm unable to remove it, but I was able to remove the balance and retrieve a manufacturer tag stuck in the track. The metal measures 28" x 1/2" and reads "27 5 LU CMC 6"

I am the home owner, a quick learner, and pretty handy person, but replacing an entire window is beyond my skill set. I hope you can help!

Thank you,
Dan

Bob from SWISCO responded:

Hey Dan. I believe this is an S765-28 Channel Balance, with a stamp option of 27-5. Please compare. If you need a new pivot shoe, it's probably the 15-046, but make sure your old shoe is 1-1/4" wide and 1/2" thick. Finally, if you need a new stop, see our 26-053A. Let me know if you have any questions.
