A home owner from Neptune beach says:
See photos for marks. These are about 20 yrs old. I need 10 of these with the winged ends. Also need the plastic caps that go on the window since that is what broke and popped these out.

Thanks for posting. This looks like either an S380-30 or S385-30 Channel Balance. The only difference between these two balances is the depth of the bottom nylon shoe, or the measurement from 'heel' to 'toe'. See the illustration below. Compare both with your old hardware to see what matches best. In either case, for a stamp option choose 2940.

As for the Top Sash Guide, these balances can work with many different kinds, so the best thing to do is to remove an undamaged one and show us clear photos of it from the front and side. We should be able to ID it from there. Many thanks.
