Hello,

At first glance, your channel balance resembles the S751-29. Please note that this assembly uses a ribbed channel, which I can't quite make out in your photos. 

If it's a match, your stamp code indicates that you would most likely need 28HD strength options for your replacements but I'd recommend confirming that your sash falls within the 22-28 lbs. range that these balances support. Thank you!
